Screen for ADDING or EDITING client information

The above data entry window opens from the CLIENT/PATIENT screen.  It opens without data if the F2 - ADD CLIENT button is pressed.  It opens with the current data for the client if the F3 - EDIT CLIENT button is pressed.  The data can be entered or edited depending on the option.
